What is MedsCheck Manage?

The MedsCheck Manage Plan provides a detailed medication review (MTM) for our most vulnerable patients:

  • Those who take more than 8 medications.
  • Those who have 2 or more chronic health conditions.
  • Those who have frequent hospitalizations or transition to new care settings.

How Does MedsCheck Manage Work?

Your consultant pharmacist conducts detailed medication reviews every time a medication is added or a transition of care occurs. Our medication reviews ensure prescriptions are appropriate, effective, and free from dangerous interactions.

When patients experience a transition of care, such as being discharged from a hospital or moving to a new care setting, their medications often change. Without proper oversight, this can lead to unnecessary medications, confusion, missed doses, and harmful side effects. Your consultant pharmacist ensures a smooth transition, reviewing any new medications, speaking with the physician about adjusting the regimen as needed, and providing clear guidance to the patient and their caregivers.
